To request a CCCSD transcript and/or graduation verification, please complete the Records Request Form below. Be sure to fill out the "special information" section if you have specific information you are seeking.
Identification verification is required for all requests. Once completed, the form and a copy of your identification can be returned by email, in person or sent through the mail.
